A systematic search was carried out in the WoS, PubMed and Scopus databases. Retrospective and prospective cohort studies, carried out until November 2020, that evaluated a population of women in childbearing age (12 to 46 years) with at least 1 induced and/or provoked abortion, including pharmacological and surgical abortion. Only studies with healthy women at the beginning of the research were included, i.e., with absence of psychiatric pathology prior to induced abortion. The quality of the included studies was measured with the Newcastle-Ottawa Scale (NOS), and for the MA random-effects models were specified using the DerSimonian & Laird method, grouping them into follow-up after abortion before and after one year. The results of the SR were measured with relative risk (RR), hazard ratio (HR), odds ratio (OR), and the chi-square test, which assessed the intensity of the statistical relationship between population and exposure. Systematic review demonstrated an OR of 1.38 (95% CI 1.14-1.68) of depression after induced abortion. Meta-analysis demonstrated a statistically significant association between depression and induced abortion when the assessment after one year was performed: OR: 1.37 (95% CI 1.09-1.71). The risks, harms and mental health consequences of induced abortion, such as depression, should be investigated and warned.</ins>

Multivariate logistic regression presented that worry (regarding cervical cancer, general health, the result of the next cytology test, and having sex) was an independent significant predictive factor for concurrent depression/anxiety in women before undergoing additional diagnostic procedures due to positive screening for cervical cancer. Multivariate logistic regression presented that urban place of residence, history of induced abortion, and sedative use were predictive factors for concurrent depression and anxiety in women after undergoing additional diagnostic procedures due to positive screening for cervical cancer.</ins>

This prospective observational study showed a correlation between certain risk factors and concurrent depression/anxiety, although it is difficult to determine causality of this association due to the limitations of the study design. Only evidence that establishes causality can definitively guide the implementation of specific procedures and interventions during cervical cancer screening aimed at reducing concurrent depression/anxiety.</blockquote>'''Li, Z., Zhang, H., Wang, Y., Liu, X., Wang, X., Feng, Q., ... & Wang, C. (2026). Association of induced abortion with mental health among rural women: an epidemiological survey study. ''Current Psychology'', ''45''(2), 189. <nowiki>https://link.springer.com/article/10.1007/s12144-025-08889-3</nowiki>'''<blockquote>Previous studies have shown that induced abortion may be a risk factor for mental problems. However, these studies have only examined the association between induced abortion and mental problems, with limited evidence regarding its long-term impact on women’s overall health. In this study, a total of 12,275 women were included in the study from the Henan Rural Cohort. Logistic regression was employed to examine the connection between induced abortion and mental problems. To further explore the potential causal relationship, Mendelian randomization (MR) study was adopted. Additionally, the Sullivan method was used to estimate healthy life expectancy (HLE) among women with different numbers of induced abortion. The findings revealed that induced abortion was associated with higher risks of depressive symptoms, anxiety symptoms, suicidal ideation and mental disorder. The MR analysis confirmed a causal link between induced abortion and depressive symptoms. Moreover, women experiencing induced abortion exhibited shorter HLE compared to those without abortion. These results suggest that induced abortion may not only affect mental health but also contribute to a reduced the HLE. Therefore, greater attention should be devoted to the mental health of women with a history of abortion. It is recommended to incorporate routine mental health screening into post-abortion care, provide comprehensive pre-abortion and post-abortion counseling, especially for high-risk groups, and enhance the training of healthcare providers.</blockquote></ins>

T'''ermination of pregnancy was associated with depression (pooled adjusted odds ratio, aOR 1.49, 95% CI 1.20–1.83) and anxiety disorder (pooled aOR 1.43, 95% CI 1.20–1.71).''' Miscarriage was associated with depression (pooled aOR 1.97, 95% CI 1.38–2.82) and anxiety disorder (pooled aOR 1.24, 95% CI 1.11–1.39). Sensitivity analyses excluding early pregnancy loss and termination reported similar results. Preterm birth was associated with depression (pooled aOR 1.37, 95% CI 1.32–1.42), anxiety disorder (pooled aOR 0.97, 95% CI 0.41–2.27) and post‐traumatic stress disorder (PTSD) (pooled aOR 1.75, 95% CI 0.52–5.89). Caesarean section was not significantly associated with PTSD (pooled aOR 2.51, 95% CI 0.75–8.37). There were few studies on other mental disorders and therefore it was not possible to perform meta‐analyses.</ins>

We found that both RPL patients and women with one pregnancy loss had significantly higher SDS and SAS scores than the control group (P = 0.006, 0.003). Furthermore, in RPL patients, those with lower education level (lower than university), lower household income (< 10,000 yuan) and history of induced abortion had significantly higher levels of depression and anxiety. Women with multiple pregnancy losses ( ≥ 3) and no live birth had significantly higher SDS scores. Women who had been married for 3 years or more had a significantly higher SAS score. Logistic regression revealed that lower education level (lower than university) was an independent risk factor for depression (adjusted OR = 1.75, 95% CI 1.10-2.77, P = 0.018) and anxiety (adjusted OR = 1.80, 95% CI 1.04-3.13, P = 0.037), and women with three or more pregnancy losses had increased odds of depression than those with two pregnancy losses (adjusted OR = 1.82, 95% CI 1.15-2.88, P = 0.012).</ins>

<ins style="font-weight: bold; text-decoration: none;">'''Conclusion:''' RPL patients are more likely to develop depression and anxiety than women with no history of pregnancy loss. Lower education level and multiple pregnancy losses (≥ 3) appear to be two independent risk factors of depression and anxiety in women with RPL. Women with one pregnancy loss also show a significant higher level for depression and anxiety. Appropriate psychological intervention can be considered for such patients.</blockquote></ins>

Epub 2022 Oct 28. PMID: 36306037.'''<blockquote>This study examined the associations between childbirth decisions in women with unintended pregnancies and long-term psychological distress. An online survey of women selected from a representative research panel was conducted in July 2021. Among participants who experienced an unintended pregnancy, the childbirth decision was categorized: (i) wanted birth, (ii) abortion, (iii) adoption, and (iv) unwanted birth. Participants who made childbirth decisions more than 1 year ago were included. ANCOVA was conducted with psychological distress (Kessler 6) as the dependent variable and education, marital status, years from the decision, age of the first pregnancy, economic situation at the unintended pregnancy, and the number of persons consulted at the unintended pregnancy as covariates. Logistic regression analysis was conducted for high distress (K6 ≥ 13) by adjusting the same covariates. A total of 47,401 respondents participated in the study. Women with an experience of unintended pregnancy experienced more than 1 year before the study were analyzed (n = 7162). Psychological distress was the lowest for wanted birth and increased for abortion, adoption, and unwanted birth. In the adjusted model, abortion was associated with lower distress scores than both adoption and unwanted birth. Compared to the wanted birth, adoption and unwanted birth showed significantly higher levels of distress (adjusted odds ratio [aOR] = 2.03 [95% CI 1.36-3.04], aOR = 1.64 [95% CI 1.04-2.58], respectively). Long-term effects on psychological distress differed according to the childbirth decisions in unintended pregnancy. Healthcare professionals should be aware of this hidden effect of unintended pregnancy experience on women's mental health.</blockquote><blockquote>'''Notes:''' This study actually showed higher rates of distress among women who aborted versus unintended pregnancies carried to term (OR 1.57; 95% CI 1.38-1.78). This was reduced to slightly less than statistical significance by controlling for covariates that may not have been appropriate. But most importantly, the results did not show statistically significantly higher levels of distress for women who wanted an abortion but did not have one or for those who placed the child for adoption compared to women who had abortions. In other words, there was no significant benefit to abortion compared to any other group but significantly higher levels of stress compared to the unintended pregnancy carried to term overall group.</blockquote>

'''[https://pubmed.ncbi.nlm.nih.gov/36306037/ Long-term influence of unintended pregnancy on psychological distress: a large sample retrospective cross-sectional study.] Sasaki N, Ikeda M, Nishi D. Arch Womens Ment Health. 2022 Dec;25(6):1119-1127. doi: 10.1007/s00737-022-01273-1. In other words, there was no significant benefit to abortion compared to any other group but significantly higher levels of stress compared to the unintended pregnancy carried to term overall group. <ins style="font-weight: bold; text-decoration: none;"> </ins>

Women with a first documentation of depression, anxiety, adjustment disorder, or somatoform disorder in one of 281 gynecological practices in Germany between January 2013 and December 2017 were included in this study (index date). Controls without depression, anxiety, adjustment disorder, or somatoform disorder were matched (1:1) to cases by age, index year, and physician. A total of 57,770 women were included in the present study. The main outcome of the study was the risk of psychiatric disorders (i.e. depression, anxiety, adjustment disorder, somatoform disorder) as a function of induced abortion, spontaneous abortion, and infertility.</ins>

<ins style="font-weight: bold; text-decoration: none;"></ins>

Induced abortion (odds ratios [ORs] ranging from 1.75 to 2.01), spontaneous abortion (ORs ranging from 2.16 to 2.60), and infertility (OR = 2.13) were positively associated with the risk of psychiatric disorders.</ins>

Within 10 years of the index date, 6.7% of the participants with induced abortions and 5.4% of those with live births were diagnosed with depression (log-rank p-value = 0.003). The respective figures were 3.4% and 2.7% for anxiety disorder (log-rank p-value = 0.255), 6.2% and 5.6% for adjustment disorder (log-rank p-value = 0.116), and 19.3% and 13.3% for somatoform disorder (log-rank p-value<0.001). Induced abortion was significantly associated with depression (hazard ratio [HR] = 1.34), adjustment disorder (HR = 1.45) and somatoform disorder (HR = 1.56), but not with anxiety disorder (HR = 1.17).</ins>
